    目的 用于探讨水平半规管视频头脉冲试验 (vHIT)实验中扫视波(CS)出现时的参数与摇头试验(HST)后产生的摇头眼震(HSN)结果相关性。方法 回顾性收集2025.3—2025.9就诊于首都医科大学附属北京安贞医院南充医院南充市中心医院患者的自发眼震(SN)、摇头眼震(HSN)及水平vHIT结果,根据SN、HSN出现情况将病例分为5组:SN(-),HSN(-),共计33例;SN(-),HSN(L),共计20例;SN(-),HSN(R),共计21例;SN(L),HSN(L),共计14例; SN(R),HSN(R),共计19例。对患者的一般情况、水平vHIT扫视波出现情况及其相关参数与SN、HSN方向的相关性进行分析。结果 各组患者平均年龄、性别、头脉冲测试左右侧总数组成,差异无统计学意义(P>0.05)。除SNL,HSNL组外,其余各组峰值速度及峰值时间差异有统计学意义(P <0.05);除SN-,HSNL外,其余各组第一扫视速度差异有统计学意义(P <0.05);将第一扫视时间与峰值时间差值取绝对值等于时间差,第一扫视速度/峰值速度得到速度比,可见除SN-,HSN-组外,其余各组时间差、速度比、CS出现概率、第一扫视时间有统计学意义(P <0.05)。结论 HSN阳性患者双侧CS出现概率更容易不对称,且HSN方向侧一般较对侧出现CS概率低;HSN阳性组CS波在HSN同向的方向侧较对侧更容易更早更高的出现。

    Abstract:

    Objective The aim of this study was to analyze the correlation between catch-up saccade (CS) parameters in the horizontal video head impulse test (vHIT) and head-shaking nystagmus (HSN) after the head-shaking test (HST), to obtain information to help improve the comprehensiveness and accuracy of the clinical evaluation of vestibular function. Methods Data on spontaneous nystagmus (SN), head-shaking nystagmus (HSN), and horizontal video head impulse test (vHIT) were retrospectively collected from patients treated at Nanchong Hospital of Capital Medical University Affiliated Beijing Anzhen Hospital (Nanchong Central Hospital) from March 2025 to September 2025. According to SN and HSN findings, patients were categorized into five groups: SN(-)/HSN(-) (n=33), SN(-)/HSN(L) (n=20), SN(-)/HSN(R) (n=21), SN(L)/HSN(L) (n=14), and SN(R)/HSN(R) (n=19).SThe general condition of patients, the occurrence of horizontal vHIT saccade and the correlation of related parameters with SN and HSN direction were subsequently analyzed. Results When HSN was positive, the occurrence rate of CS during bilateral head impulse test was statistically significant (P <0.05). The time to peak head movement was significantly different between all the groups (P <0.05). Further, except for the SNL/HSNL group, the peak velocities were significantly different (P <0.05); and, except for the SN-/HSNL group, the first saccade velocities in the other groups were statistically significant (P <0.05). The absolute value of the difference between the first scanning time and peak time was equal to the time difference, and the speed ratio between the first scanning speed and peak speed was obtained. Overall, these results indicate that, except for the SN-/HSN- group, the time difference, the speed ratio, the probability of CS occurrence and the first scanning time all showed significant differences between groups (P <0.05). Conclusion The probability of bilateral CS occurrence in HSN-positive patients is more likely to be asymmetrical, while the probability of CS occurrence on the direction side of the HSN is generally lower than that on the opposite side. The CS wave in the HSN-positive group was more likely to appear earlier and higher on the direction of the HSN than on the opposite side.
