    目的:探讨定喘汤加减联合NPPV应用于慢性阻塞性肺疾病(COPD)合并呼吸衰竭(RF)的治疗效果及临床安全性。方法:选取2024年1月至2025年12月期间于本院治疗的AECOPD合并RF患者86例作为研究对象。采用随机数字表法将患者分为对照组和研究组,每组各43例。对照组采用单纯无创正压通气治疗。研究组在对照组基础上联合定喘汤加减治疗。分析对比两组的中医证候积分、动脉血气指标[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)]、肺功能[第1秒用力呼气容积(FEV1)、用力肺活量实测值(FVC)以及FEVl /FVC]、不良反应以及疗效。结果:研究组的临床治疗总有效率显著高于对照组(P<0.05)。治疗2周时,两组的中医证候积分均比治疗前显著降低,且研究组的中医证候积分显著低于对照组(P<0.05)。治疗2周时,两组的PaO2比治疗前显著提高、PaCO2比治疗前显著降低,且观察组的PaO2、PaCO2的改善幅度均显著大于对照组(P<0.05)。治疗2周时,两组的FEV1、FVC、FEVl/FVC各项肺功能均比治疗前显著提高,且研究组的各项肺功能均显著高于对照组(P<0.05)。两组治疗期间的不良反应发生率无显著差异(P>0.05)。结论:定喘汤加减联合NPPV治疗COPD合并RF,可提高临床疗效,缓解中医证候,改善动脉血气指标及肺功能,且安全性好。

    Abstract:

    Objective: To explore the therapeutic effect and clinical safety of modified Dingchuan Decoction combined with NPPV in the treatment of chronic obstructive pulmonary disease (COPD) complicated with respiratory failure (RF). Method: 86 patients with AECOPD combined with RF who were treated in our hospital from January 2024 to December 2025 were selected as the research subjects. The patients were divided into the control group and the study group by random number table method, with 43 cases in each group. The control group was treated with simple non-invasive positive pressure ventilation. The research group was treated with modified Dingchuan Decoction on the basis of the control group. The TCM syndrome scores, arterial blood gas indicators [arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2)], pulmonary function [forced expiratory volume in one second (FEV1), measured forced vital capacity (FVC), and FEVl /FVC], adverse reactions and therapeutic effects of the two groups were analyzed and compared. Result: The total effective rate of clinical treatment in the study group was significantly higher than that in the control group (P < 0.05). At 2 weeks of treatment, the TCM syndrome scores of both groups were significantly lower than those before treatment, and the TCM syndrome score of the study group was significantly lower than that of the control group (P < 0.05). At 2 weeks of treatment, PaO2 in both groups was significantly increased compared with that before treatment, and PaCO2 was significantly decreased compared with that before treatment. Moreover, the improvement amplitudes of PaO2 and PaCO2 in the observation group were significantly greater than those in the control group (P < 0.05). At 2 weeks of treatment, the lung functions of FEV1, FVC, and FEVl/FVC in both groups were significantly improved compared with those before treatment, and the lung functions of the study group were significantly higher than those of the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups during the treatment period (P > 0.05). Conclusion: Modified Dingchuan Decoction combined with NPPV in the treatment of COPD complicated with RF can improve clinical efficacy, alleviate TCM syndromes, enhance arterial blood gas indicators and lung function, and has good safety.
