    目的:探究胃功能三项胃蛋白酶原I(Pepsinogen I, PG I)、PG II、胃泌素17(G 17)与老年胃食管反流病(Gastroesophageal reflux disease, GERD)患者症状评分的关系。方法:选择2024年1月至2024年12月医院收治的GERD患者106例作为GERD组,另选取同期健康体检的老年人60例作为对照组,根据GERD症状评分将GERD组患者分为轻度(症状评分<8分)、中度(8分≤症状评分≤15分)、重度(症状评分>15分)。比较不同病情严重程度的患者PG I、PG II、G 17浓度。Spearman相关性分析法分析老年GERD患者胃功能三项与GERD症状评分的关系。结果:相比对照组,GERD组患者PG II水平更高,PG I、G 17水平更低(P<0.05)。随着病情加重,GERD轻度、中度和重度组三组PG II水平均上升,PG I、G 17水平水平均下降(P<0.05)。Spearman相关性分析结果显示,老年GERD患者症状评分与PG II水平呈显示正相关(P<0.05),与PG I、G 17水平呈显著负相关。结论:老年GERD患者PGII水平随着症状评分增加而上升、PGI、G 17水平则随症状评分上升而下降,三者均与老年GERD患者症状评分存在显著相关性,值得临床重视。

    Abstract:

    Objective: To explore the relationship between the three indicators of gastric function (pepsinogen I (PG I), pepsinogen II (PG II), and gastrin-17 (G-17))and symptom scores in elderly patients with gastroesophageal reflux disease (GERD). Methods: We selected 106 elderly patients with GERD who were admitted to the hospital from January to December 2024 as the GERD group, and 60 healthy elderly individuals who received health check-ups during the same period as the control group. Based on symptom scores, patients in the GERD group were further divided into the mild group (symptom scores <8), the moderate group (8 ≤ symptom scores ≤15), and the severe group (symptom scores >15). The levels of PG I, PG II, and G-17 were compared among patients with different severities. Spearman correlation analysis was used to explore the relationship between the three indicators of gastric function and symptom scores in elderly patients with GERD. Results: Compared with the control group, the GERD group had higher PG II levels and lower PG I and G-17 levels (P<0.05). As the patient"s condition worsened, PG II levels rose, while PG I and G-17 levels decreased across the mild, moderate, and severe groups (P<0.05). Spearman correlation analysis revealed that symptom scores in elderly patients with GERD were positively correlated with PG II level (P<0.05) and negatively correlated with PG I and G-17 levels (P<0.05). Conclusion: In elderly patients with GERD, PG II levels increase, while PG I and G-17 levels decrease with the increase of symptom scores. All three indicators of gastric function show significant correlations with symptom scores in elderly patients with GERD, which deserves clinical attention.
