双相情感障碍女性患者心肌酶水平与情绪发作频率、严重程度的相关性及影响因素探讨

    目的:探讨双相情感障碍(Bipolar Disorder, BD)女性患者心肌酶水平与情绪发作频率、严重程度的相关性,并分析影响心肌酶异常的关键因素。方法:回顾性纳入2022年1月至2025年8月我院收治的80例女性BD患者,根据情绪发作类型分为躁狂发作组(32例)、抑郁发作组(30例)、混合发作组(18例)。检测所有患者血清心肌酶谱,包括肌酸激酶(Creatine Kinase, CK)、肌酸激酶同工酶MB(CK-MB)、乳酸脱氢酶(Lactate Dehydrogenase, LDH)、α-羟丁酸脱氢酶(α-Hydroxybutyrate Dehydrogenase, α-HBDH)。评估其汉密尔顿抑郁量表-17项(17-item Hamilton Depression Rating Scale, HAMD-17)评分、杨氏躁狂评定量表(Young Mania Rating Scale, YMRS)评分,并记录近1年内情绪发作总次数。分析心肌酶水平与情绪发作频率、严重程度的相关性及影响因素。结果:躁狂发作组CK、CK-MB水平均显著高于抑郁发作组和混合发作组,混合发作组LDH、α-HBDH水平最高(P<0.05);抑郁发作组HAMD-17评分最高,躁狂发作组YMRS评分最高,混合发作组情绪发作总次数最多(P<0.05);血清CK、CK-MB水平与情绪发作总次数呈正相关,CK、CK-MB、LDH、α-HBDH与YMRS评分呈正相关(P<0.05),但与HAMD-17评分无显著相关性(P>0.05);高CK水平组患者的YMRS评分及近1年内情绪发作总次数均显著高于低CK水平组(P<0.05);多元线性逐步回归分析结果显示,YMRS评分和情绪发作总次数是影响BD患者血清CK水平的独立影响因素(P<0.05)。结论:在BD女性患者中,躁狂症状的严重程度和情绪发作频率是心肌酶水平,尤其是CK水平升高的独立相关因素。监测心肌酶,可能有助于评估患者的躁狂严重程度和疾病活动性。

    Abstract:

    Objective: To explore the correlation between myocardial enzyme levels and the frequency and severity of emotional attacks in female patients with bipolar disorder (BD), and to analyze the key factors affecting myocardial enzyme abnormalities. Methods: A retrospective study was conducted on 80 female BD patients admitted to our hospital from January 2022 to August 2025. They were divided into manic episode group (32 cases), depressive episode group (30 cases), and mixed episode group (18 cases) based on the type of emotional attack. Detect the serum myocardial enzyme profiles of all patients, including creatine kinase (CK), creatine kinase MB (CK-MB), lactate dehydrogenase (LDH), and alpha hydroxybutyrate dehydrogenase (alpha HBDH). Evaluate their Hamilton Depression Rating Scale (HAMD-17) scores and Young Mania Rating Scale (YMRS) scores, and record the total number of emotional episodes in the past year. Analyze the correlation and influencing factors between myocardial enzyme levels and the frequency and severity of emotional attacks. Results: The levels of CK and CK-MB in the manic episode group were significantly higher than those in the depressive episode group and the mixed episode group, with the highest levels of LDH and α - HBDH in the mixed episode group (P<0.05); The HAMD-17 score was highest in the depressive episode group, the YMRS score was highest in the manic episode group, and the total number of emotional episodes was highest in the mixed episode group (P<0.05); The levels of serum CK and CK-MB are positively correlated with the total number of emotional attacks. CK, CK-MB, LDH, and α - HBDH are positively correlated with YMRS scores (P<0.05), but not significantly correlated with HAMD-17 scores (P>0.05); The YMRS score and total number of emotional attacks in the high CK level group were significantly higher than those in the low CK level group within the past year (P<0.05); The results of multiple linear stepwise regression analysis showed that YMRS score and total number of emotional attacks were independent factors affecting the serum CK level of BD patients (P<0.05). Conclusion: In female patients with BD, the severity of manic symptoms and frequency of emotional episodes are independent correlated factors with elevated levels of myocardial enzymes, especially CK levels. Monitoring myocardial enzymes may help assess the severity of mania and disease activity in patients.
