    目的:本研究旨在评估磁共振波谱成像(Magnetic Resonance Spectroscopy, MRS)在胶质瘤、脑膜瘤及脑转移瘤鉴别诊断中的价值。方法:共纳入102例经病理证实的颅内肿瘤患者,分为胶质瘤组(n=44)、脑膜瘤组(n=30)和转移瘤组(n=28)。所有患者均接受3.0T 磁共振成像(Magnetic Resonance Imaging, MRI)及MRS检查,包括单体素与多体素波谱序列,分析肿瘤实质区及瘤周水肿区的代谢物比值、胆碱化合物/N-乙酰天门冬氨酸( Choline to N-Acetylaspartate Ratio ,Cho/NAA)、胆碱化合物/肌酸(Choline to Creatine Ratio,Cho/Cr)、N-乙酰天门冬氨酸/肌酸 (N-Acetylaspartate to Creatine Ratio ,NAA/Cr)、(脂质+乳酸)/肌酸(Combined Lipid and Lactate to Creatine Ratio,Lip+Lac/Cr)及瘤周相对胆碱比值(Relative Peri-tumoral Choline to Creatine Ratio ,rCho/Cr)。结果:三组肿瘤在实质区各代谢物比值上差异显著(P<0.001)。胶质瘤组Cho/NAA和Cho/Cr最高,转移瘤组(Lip+Lac)/Cr最高,脑膜瘤组NAA/Cr最高。瘤周水肿区rCho/Cr在胶质瘤组显著高于其余两组(P<0.001)。受试者工作特征(Receiver Operating Characteristic, ROC)曲线分析表明,瘤周rCho/Cr和肿瘤Cho/NAA在鉴别高级别胶质瘤(High-Grade Glioma, HGG)与单发转移瘤(Solitary Metastasis, SM)方面具有较高诊断效能(曲线下面积(Area Under the Curve, AUC)分别为0.876和0.892)。两名医师测量结果一致性良好(组内相关系数(Intraclass Correlation Coefficient, ICC)>0.85)。结论:MRS能有效反映不同类型脑肿瘤的代谢特征,尤其在瘤周水肿区rCho/Cr分析中表现出优越的鉴别诊断价值,可为临床术前鉴别提供重要影像学依据。

    Abstract:

    Objective: This study aimed to evaluate the value of magnetic resonance spectroscopy (MRS) in the differential diagnosis of gliomas, meningiomas, and brain metastatic tumors.Methods: A total of 102 patients with pathologically confirmed intracranial tumors were enrolled and divided into three groups: the glioma group (n=44), the meningioma group (n=30), and the metastatic tumor group (n=28). All patients underwent 3.0T magnetic resonance imaging (MRI) and MRS examinations, including single-voxel and multi-voxel spectroscopy sequences. The metabolite ratios (choline compounds/N-acetylaspartate [Cho/NAA], choline compounds/creatine [Cho/Cr], N-acetylaspartate/creatine [NAA/Cr], (lipid+lactate)/creatine [(Lip+Lac)/Cr], and peritumoral relative choline ratio [rCho/Cr]) in the tumor parenchyma and peritumoral edema regions were analyzed.Results: There were significant differences in the metabolite ratios of the parenchyma region among the three tumor groups (P<0.001). The glioma group had the highest Cho/NAA and Cho/Cr ratios, the metastatic tumor group had the highest (Lip+Lac)/Cr ratio, and the meningioma group had the highest NAA/Cr ratio. The rCho/Cr ratio in the peritumoral edema region of the glioma group was significantly higher than that of the other two groups (P<0.001). Receiver operating characteristic (ROC) curve analysis showed that peritumoral rCho/Cr and tumor Cho/NAA had high diagnostic efficacy in distinguishing high-grade gliomas (HGG) from solitary metastatic tumors (SM) (with areas under the curve (AUC) of 0.876 and 0.892, respectively). The consistency of the measurement results between the two physicians was good (intraclass correlation coefficient (ICC) > 0.85).Conclusion: MRS can effectively reflect the metabolic characteristics of different types of brain tumors, and exhibits superior differential diagnostic value especially in the analysis of rCho/Cr in the peritumoral edema region. It can provide important imaging evidence for clinical preoperative differentiation.
