    目的:观察艾司氯胺酮辅助罗哌卡因股神经阻滞对膝关节置换术后镇痛效果及认知功能的影响。方法:选取2024年8月至2025年8月期间行膝关节置换术的80例患者,随机平均分为观察组和对照组,对照组予以罗哌卡因股神经阻滞,观察组予以艾司氯胺酮辅助罗哌卡因股神经阻滞,对比手术后感觉和运动阻滞持续时间,术后2h、8h和24h的静息、活动状态下VAS评分(visual analogue scale,视觉模拟疼痛评分量表)、认知功能评估MoCA评分(Montreal Cognitive Assessment, 蒙特利尔认知评分量表)以及不良反应发生率。结果:手术后,观察组相比对照组,感觉和运动阻滞持续时间均更高(P<0.05);手术后,观察组相比对照组,各时间段VAS评分均更低(P<0.05);观察组相比对照组,术后1d和3d,MoCA评分更高(P<0.05),术后7d观察组和对照组MoCA评分无明显差异(P>0.05);手术与住院期间,两组患者出现关节僵硬、血管损伤、深静脉血栓和创口感染的概率无明显差异(P<0.05)。结论:艾司氯胺酮辅助罗哌卡因股神经阻滞应用于膝关节置换术可以有效提高感觉与运动阻滞时间,减轻患者疼痛,有益于术后认知功能保留并具有良好的安全性。

    Abstract:

    Objective: To explore the influences of esketamine combined with ropivacaine for femoral nerve block on analgesic effect and cognitive function after knee arthroplasty. Methods: A total of 80 patients undergoing knee arthroplasty were enrolled between August 2024 and August 2025.They were divided into observation group and control group. The control group was given ropivacaine, while observation group was given esketamine combined with ropivacaine for femoral nerve block. The postoperative duration of sensory and motor block, scores of visual analogue scale (VAS) under resting and motion states at 2h, 8h and 24h after surgery, scores of Montreal Cognitive Assessment (MOCA) and the incidence of adverse reactions were compared. Results: After surgery, the duration of sensory and motor block in observation group was longer than that in control group (P<0.05). At 2h, 8h and 24h after surgery,VAS scores under resting and motion states in observation group were significantly lower than those in control group (P<0.05). At 1d and 3d after surgery, MoCA scores in observation group were higher than those in control group (P<0.05), but there was no significant difference in MoCA score between the two control groups at 7d after surgery (P>0.05). During operation and hospitalization, There was no significant difference in the probability of joint stiffness, vascular injury, deep vein thrombosis and wound infection between the two groups (P>0.05). Conclusion: The application of esketamine combined with ropivacaine for femoral nerve block in knee arthroplasty can effectively prolong the duration of sensory and motor block, and relieve pain, which is beneficial to preserve postoperative cognitive function, with good safety.
