    目的:分析颈动脉超声在评估急性脑梗死(Acute cerebral infarction, ACI)颈动脉狭窄及预后中的应用价值。方法:纳入2022年8月至2025年8月宝丰县中医院ACI患者102例,所有患者均于入院72 h内接受双侧颈动脉超声检查,测量责任病灶侧颈动脉的内中膜厚度(Intima-media thickness, IMT)、斑块形态学特征(稳定/易损斑块)及狭窄程度,采集并分析最狭窄处血流动力学参数。根据发病后90d内改良Rankin量表(modified Rankin Scale, mRS)评分结果分为预后良好组(mRS≤2分,62例)和预后不良组(mRS>2分,40例),比较两组临床资料与颈动脉超声参数,采用Pearson相关性分析探讨入院美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale, NIHSS)评分与颈动脉超声参数的相关性,通过多因素Logistic回归模型,筛选影响ACI患者90d功能预后的独立影响因素。结果:预后不良组IMT厚于预后良好组P<0.05),易损斑块检出率,重度狭窄构成比及责任病灶侧颈内动脉缩期峰值流速(Peak systolic velocity, PSV)、舒张末期血流速(End-diastolic velocity, EDV)高于预后良好组(P<0.05);Pearson相关性分析显示,患者入院NIHSS评分与IMT、PSV、EDV均呈显著正相关(P<0.05);多因素Logistic回归分析显示,高龄、高入院NIHSS评分、存在颈动脉重度狭窄以及存在易损斑块是ACI患者90 d不良预后的独立危险因素。结论:颈动脉超声参数与ACI患者入院时神经功能缺损程度密切相关,其中颈动脉重度狭窄和易损斑块是预测短期功能预后的重要指标。

    Abstract:

    Objective: To analyze the application value of carotid ultrasound in the evaluation of carotid artery stenosis and prognosis in patients with acute cerebral infarction (ACI). Methods: A total of 102 patients with ACI in Baofeng County Hospital of Traditional Chinese Medicine from August 2022 to August 2025 were enrolled. All patients underwent bilateral carotid ultrasonography within 72 hours of admission. The intima-media thickness (IMT), plaque morphological characteristics (stable/vulnerable plaques) and stenosis degree of the carotid artery on the responsible lesion side were measured. The hemodynamic parameters of the narrowest stenosis site were collected and analyzed. According to the modified Rankin Scale (mRS) score within 90 days after onset, the patients were divided into good prognosis group (mRS≤2 points, 62 cases) and poor prognosis group (mRS>2 points, 40 cases). The clinical data and carotid ultrasound parameters were compared between the two groups. Pearson correlation analysis was used to explore the correlation between National Institutes of Health Stroke Scale (NIHSS) score at admission and carotid ultrasound parameters. Multivariate Logistic regression model was used to screen the independent influencing factors affecting the 90-day poor prognosis in ACI patients. Results: IMT in poor prognosis group was thicker than that in good prognosis group (P<0.05), and the detection rate of vulnerable plaques, proportion of severe stenosis, peak systolic velocity (PSV) and end-diastolic velocity (EDV) of the internal carotid artery in the responsible lesion side were higher than those in good prognosis group (P<0.05). Pearson correlation analysis showed that NIHSS score at admission was positively correlated with IMT, PSV and EDV (P<0.05). Multivariate Logistic regression analysis showed that advanced age, high NIHSS score at admission, presence of severe carotid artery stenosis and presence of vulnerable plaques were independent risk factors of 90-day poor prognosis in ACI patients. Conclusion: Carotid ultrasound parameters are closely related to the neurological deficit degree in patients with ACI at admission. Severe carotid artery stenosis and vulnerable plaques are important indicators for predicting short-term poor prognosis.
