    目的:观察肺癌患者在紫杉醇联合顺铂(Paclitaxel + Cisplatin, TP)化疗方案上加用扶正祛邪方对其免疫功能、毒副反应的影响。方法:遴选舞钢市人民医院肿瘤内科2023年01至2024年9月收治的114例肺癌患者,依据随机数字表法将其分为常规组和中药组,每组57例。常规组接受TP化疗,中药组在常规组基础上加用扶正祛邪汤加减。观察两组中医症候疗效,治疗前、后的肿瘤标志物和免疫功能指标水平,并统计治疗期间患者毒副反应情况。结果:中药组的治疗总有效率显著高于常规组(P < 0.05);治疗5个疗程后,两组癌胚抗原(Carcinoembryonic Antigen, CEA)、糖类抗原125(Carbohydrate Antigen 125, CA125)和神经元特异性烯醇化酶(Neuron-Specific Enolase, NSE)均显著下降(P < 0.05),且中药组在CEA和CA125水平低于常规组(P < 0.05);治疗5个疗程后,中药组簇分化抗原4阳性(Cluster of Differentiation 4 Positive, CD4+)T细胞亚群和CD4+/CD8+比值较治疗前显著升高(P < 0.05),且中药组高于常规组(P < 0.05);两组治疗5个疗程后CD8+T细胞亚群较治疗前下降(P < 0.05),但中药组水平较常规组更高(P < 0.05);治疗5个疗程内,两组均无死亡病例(5级),中医组在恶心呕吐、贫血和白细胞减少的严重程度低于常规组(P < 0.05)。结论:在TP化疗方案基础上加用扶正祛邪方可协同增强肺癌治疗疗效,有效降低肿瘤标志物表达,改善机体免疫功能,且安全性较好。

    Abstract:

    Objective: To observe the influence of Fuzheng Quxie decoction on the basis of paclitaxel + cisplatin (TP) chemotherapy regimen on immune function and toxic and side effects in patients with lung cancer. Methods: A total of 114 patients with lung cancer admitted to department of medical oncology of Wugang People’s Hospital from January 2023 to September 2024 were selected and divided into conventional group and traditional Chinese medicine group according to the random number table method, with 57 cases in each group. The conventional group received TP chemotherapy, while the traditional Chinese medicine group was added with modified Fuzheng Quxie decoction on the basis of the conventional group. The efficacy of TCM symptoms, and levels of tumor markers and immune function indexes before and after treatment were observed in the two groups. The toxic and side effects of patients during treatment were counted. Results: The total effective rate of treatment in traditional Chinese medicine group was significantly higher than that in conventional group (P<0.05). After 5 courses of treatment, the levels of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125) and neuron-specific enolase (NSE) in the two groups were significantly decreased (P<0.05), and the levels of CEA and CA125 were lower in traditional Chinese medicine group than those in conventional group (P<0.05). After 5 courses of treatment, the cluster of differentiation 4 positive (CD4+) T cell subset and CD4+/CD8+ ratio in traditional Chinese medicine group were significantly enhanced compared with those before treatment (P<0.05), and the indexes were higher in traditional Chinese medicine group than those in conventional group (P<0.05). The CD8+ T cell subset in both groups after 5 courses of treatment was lower than that before treatment (P<0.05), but the level in traditional Chinese medicine group was higher than that in conventional group (P<0.05). Within 5 courses of treatment, there were no deaths (grade 5) in both groups. The severities of nausea and vomiting, anemia and leukopenia in traditional Chinese medicine group were milder than those in conventional group (P<0.05). Conclusion: Fuzheng Quxie decoction on the basis of TP chemotherapy regimen can synergistically enhance the therapeutic efficacy on lung cancer, effectively reduce the expressions of tumor markers, and improve the body’ s immune function, with good safety.
