    目的:研究通心络胶囊与达格列净联用治疗老年2型糖尿病(Type 2 diabetes mellitus,T2DM)合并慢性心力衰竭(Chronic heart failure,CHF)的临床疗效及安全性。方法:本研究纳入2022年5月至2025年4月期间我院老年病科诊治的老年T2DM合并CHF患者82例,随机分为两组(n=41)。所有受试者均接受标准化基础治疗,对照组另口服达格列净(5 mg·次-1,1次·d-1),观察组在此基础上联合口服通心络胶囊(3粒·次-1,3次·d-1)。两组均连续治疗6 m,评估两组临床疗效。治疗前、治疗6 m后,采用葡萄糖氧化酶法检测空腹血糖(Fasting plasma glucose,FPG)、餐后2 h血糖(2-hour postprandial glucose,2 hPG),采用高效液相色谱法检测糖化血红蛋白(Glycated hemoglobin A1c,HbA1c),采用彩色多普勒检测左室射血分数(Left ventricular ejection fraction,LVEF)、左室收缩末期内径(Left ventricular end-systolic diameter,LVESD)、左室舒张末期内径(Left ventricular end-diastolic diameter,LVEDD)、室间隔厚度(Interventricular septal thickness,IVST),采用化学发光法检测血清N末端B型脑钠肽前体(N-terminal pro-b-type natriuretic peptide,NT-proBNP),采用酶联免疫法检测血清白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、可溶性生长刺激表达基因2蛋白(Soluble suppression of tumorigenicity 2,sST2),采用糖尿病特异性生存质量量表(Diabetes-specific quality of life scale,DSQL)、明尼苏达心力衰竭生活问卷(Minnesota living with heart failure questionnaire,MLHFQ)评估生活质量。比较两组不良反应情况。结果:观察组总有效率与对照组相比显著升高(97.56% vs. 80.49%,P<0.05)。治疗后,观察组FPG、2 hPG、HbA1c、LVESD、LVEDD、IVST、血清NT-proBNP、IL-6、TNF-α、sST2水平以及DSQL、MLHFQ评分均显著低于对照组(P<0.05),LVEF显著高于对照组(P<0.05)。两组不良反应率无显著差异(P>0.05)。结论:通心络胶囊联用达格列净可改善老年T2DM合并CHF患者的血糖、心功能及炎症指标,提升生活质量,疗效确切。

    Abstract:

    Objective: To investigate the clinical efficacy and safety of Tongxinluo capsule combined with dapagliflozin in the treatment of elderly patients with type 2 diabetes mellitus (T2DM) complicated with chronic heart failure (CHF). Methods: A total of 82 elderly patients with T2DM complicated with CHF who were diagnosed and treated in the geriatrics department of our hospital from May 2022 to April 2025 were included in this study and randomly divided into two groups (n=41). All subjects received standardized basic treatment. The control group was orally administered dapagliflozin (5 mg·time-1, 1 time·d-1), while the observation group was orally administered Tongxinluo capsules (3 capsules·time-1, 3 times·d-1) on this basis. Both groups were treated continuously for 6 m, and the clinical efficacy of the two groups was evaluated. Before treatment and 6 m after treatment, fasting plasma glucose (FPG) and 2-hour postprandial glucose (2 hPG) were detected by the glucose oxidase method, glycated hemoglobin A1c (HbA1c) was detected by high performance liquid chromatography, the left ventricular ejection fraction (LVEF), left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD) and interventricular septal thickness (IVST) were measured by color doppler, serum N-terminal pro-b-type natriuretic peptide (NT-proBNP) was detected by chemiluminescence method, the serum levels of interleukin-6 (IL-6), tumor necrosis factor - α (TNF - α) and soluble suppression of tumorigenicity 2 (sST2) were detected by enzyme linked immunosorbent assay, the diabetes-specific quality of life scale (DSQL) and minnesota living with heart failure questionnaire (MLHFQ) were used to evaluate the quality of life. The adverse reactions of the two groups were compared. Results: The total effective rate of the observation group was significantly higher than that of the control group (97.56% vs. 80.49%, P<0.05). After treatment, the FPG, 2 hPG, HbA1c, LVESD, LVEDD, IVST, serum NT-proBNP, IL-6, TNF-α, sST2 levels and DSQL and MLHFQ scores in the observation group were significantly lower than those in the control group (P<0.05), and LVEF was significantly higher than that in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Tongxinluo capsule combined with dapagliflozin can improve blood glucose, cardiac function and inflammatory indexes, and improve the quality of life in elderly patients with T2DM complicated with CHF.
