    目的:2型糖尿病(Type 2 diabetes mellitus,T2DM)患者D-二聚体(D-Dimer,D-D)、促甲状腺激素(Thyroid stimulating hormone,TSH)水平变化与血管并发症的相关性。方法:选取2024年1月-2025年1月在本院收治90例T2DM患者,30例单纯T2DM患者纳入A组,30例T2DM合并微血管病变纳入B组,30例T2DM合并大血管病变纳入C组。比较各组临床基本资料、生化指标、D-D及TSH指标;采用受试者工作特征(Receiver operating characteristic,ROC)曲线评估D-D及TSH对T2DM患者发生血管并发症的评估价值。结果:3组患者性别无统计学差异(P>0.05);B、C组年龄高于A组,B组BMI低于A组,C组糖尿病病程高于A、B组,差异显著(P<0.05);B组、C组患者FBG、HbAlc均高于A组,C组患者FBG、HbAlc均高于B组,差异显著(P<0.05)。B组、C组患者D-D、TSH均高于A组,C组患者D-D、TSH均高于B组,差异显著(P<0.05)。D-D、TSH水平及联合检测诊断血管并发症患者的AUC分别为0.708、0.683、0.749(P<0.05)结论:T2DM患者的D-D和TSH水平与血管并发症密切相关,D-D和TSH可能作为预测血管并发症的生物标志物,为临床早期干预提供参考。

    Abstract:

    Objective: To explore the correlation between changes of D-Dimer (D-D) and thyroid stimulating hormone (TSH) levels and vascular complications in patients with type 2 diabetes mellitus (T2DM). Methods: A total of 90 patients with T2DM admitted to the hospital were enrolled between January 2024 and January 2025, including 30 cases with simple T2DM in group A, 30 cases with T2DM and microvascular lesions in group B and 30 cases with T2DM and macrovascular lesions in group C. The clinical basic data, biochemical indexes, D-D and TSH in different groups were compared. The evaluation value of D-D and TSH in vascular complications was assessed by receiver operating characteristic (ROC) curves. Results: There was no significant difference in gender among the three groups (P<0.05). The age of patients in groups B and C was older than that in group A, BMI in group B was lower than that in group A, and course of diabetes mellitus in group C was longer than that in groups A and B, the differences were statistically significant (P<0.05). In group A, group B and group C, FBG and HbAlc were gradually increased (P<0.05), levels of D-D and TSH were also gradually increased (P<0.05). AUC values of D-D, TSH and combined detection in the diagnosis of vascular complications were 0.708, 0.683 and 0.749, respectively (P<0.05). Conclusion: The levels of D-D and TSH are closely related to vascular complications in patients with T2DM. D-D and TSH may be applied as biomarkers to predict vascular complications, which provides reference for early clinical intervention.
