机器人辅助训练联合针刺治疗对偏瘫患者下肢运动功能康复及步行效率的提升效果

    摘要:

    目的:探讨机器人辅助训练联合针刺治疗对偏瘫患者的应用效果,并分析对下肢运动功能康复及步行效率的影响。方法:选取2022年3月~2025年3月西安市第九医院98例脑卒中后偏瘫患者进行前瞻性研究,应用抽签法进行分组,49例为对照组,采取常规康复训练,49例为试验组在常规康复训练基础上增加机器人辅助训练联合针刺。比较两组治疗前后下肢运动功能,步行效率,神经电生理及影像学指标及日常生活能力。结果:治疗后,试验组与对照组Fugl-Meyer运动功能评定量表下肢部分(Fugl Meyer Motor Function Assessment Scale for Lower Limbs,FMA-LE)、Berg平衡量表(Berg balance scale,BBS)评分升高,改良Ashworth量表(Improved Ashworth Scale,MAS)分级降低,且试验组更优(P<0.05);治疗后,试验组与对照组6分钟步行距离、步速及功能性步行量表(Functional Walking Scale,FAC)分级升高,且试验组更高(P<0.05);治疗后,试验组与对照组运动诱发电位(Motor evoked potential,MEP)潜伏期降低,MEP波幅、皮质脊髓束各向异性分数(Score of anisotropy of corticospinal tract,FA)值升高,且试验组更优(P<0.05);治疗后,试验组与对照组Barthel指数、功能独立性评定量表(Functional Independence Assessment Scale,FIM)评分升高,且试验组更高(P<0.05)。结论:机器人辅助训练联合针刺治疗脑卒中偏瘫可改善患者下肢运动功能,提升步行效率,并改善患者神经电生理及影像学指标,提升日常生活能力。

    Abstract:

    Objective: To explore the application effect of robot-assisted training combined with acupuncture therapy on hemiplegic patients and analyze its impact on the rehabilitation of lower limb motor function and walking efficiency.Methods: A prospective study was conducted on 98 patients with post-stroke hemiplegia admitted to Xi"an Ninth Hospital from March 2022 to March 2025. Patients were randomly assigned into two groups using the lottery method, with 49 patients in the control group receiving conventional rehabilitation training and 49 patients in the experimental group receiving robot-assisted training combined with acupuncture in addition to conventional rehabilitation training. Lower limb motor function, walking efficiency, neuroelectrophysiological and imaging indicators, as well as activities of daily living (ADL) were compared between the two groups before and after treatment.Results: After treatment, both groups showed increased scores in the lower limb section of the Fugl-Meyer Motor Function Assessment Scale (FMA-LE) and the Berg Balance Scale (BBS), along with a decreased classification on the Improved Ashworth Scale (MAS), with the experimental group demonstrating superior outcomes (P < 0.05). Additionally, both groups exhibited improvements in the 6-minute walk distance, walking speed, and Functional Walking Scale (FAC) classification, with the experimental group achieving higher values (P < 0.05). Regarding neuroelectrophysiological and imaging indicators, both groups demonstrated reduced latency and increased amplitude of motor evoked potentials (MEP), as well as elevated fractional anisotropy (FA) values of the corticospinal tract, with the experimental group showing more pronounced improvements (P < 0.05). Furthermore, both groups exhibited increased scores on the Barthel Index and the Functional Independence Assessment Scale (FIM), with the experimental group achieving higher scores (P < 0.05).Conclusion: Robot-assisted training combined with acupuncture therapy can improve lower limb motor function, enhance walking efficiency, optimize neuroelectrophysiological and imaging indicators, and elevate activities of daily living in patients with post-stroke hemiplegia.
