    摘要:

    目的:评价畸形率和DNA碎片指数对辅助生殖技术胚胎发育情况与妊娠结局的影响。方法:本研究为回顾性分析,选择医院2022.01-2024.01本院实施辅助生殖技术的108对夫妇,根据男方DNA碎片水平分为两组,A组(DFI<25%)(79例)、B组(DFI≥25%)(29例),比较两组的受精率、优胚率、临床妊娠率与早期流产率。同时根据精子形态学分析结果,对比精子畸形组与正常形态精子组的妊娠结局。结果:A组受精率、卵裂率及优质胚胎率高于B组(P<0.05);A组早期流产率低于B组(P<0.05),妊娠率、持续妊娠率高于B组(P<0.05);正常形态精子组的妊娠率高于精子畸形组(P<0.05)。结论:高水平的精子DNA碎片指数与畸形率是导致辅助生殖技术妊娠不良结局的主要影响因素,针对拟行辅助生殖技术助孕的夫妇,需重视进行男性精子畸形率与DNA碎片指数的的检测,可有利于早期开展个性化干预,并达到改善预后的效果。

    Abstract:

    Objective: To evaluate the effects of malformation rate and DNA fragmentation index on the development of assisted reproductive technology embryos and pregnancy outcomes. Method: This study is a retrospective analysis of 108 couples who underwent assisted reproductive technology in our hospital from January 2022 to January 2024. The cycles were divided into two groups based on the male"s DNA level: Group A (DFI<25%) (79 cases) and Group B (DFI ≥ 25%) (29 cases). The fertilization rate, excellent embryo rate, clinical pregnancy rate, and early miscarriage rate were compared between the two groups. At the same time, based on the analysis of sperm morphology, compare the pregnancy outcomes between the abnormal sperm group and the normal morphology sperm group. Result: The fertilization rate, cleavage rate, and high-quality embryo rate in Group A were higher than those in Group B (P<0.05); The early miscarriage rate in Group A was lower than that in Group B (P<0.05), while the pregnancy rate and sustained pregnancy rate were higher than those in Group B (P<0.05); The pregnancy rate of the group with normal sperm morphology was higher than that of the group with abnormal sperm morphology (P<0.05). Conclusion: High levels of sperm DNA fragmentation index and abnormality rate are the main influencing factors for adverse pregnancy outcomes of assisted reproductive technology. For couples planning to undergo assisted reproductive technology, it is important to pay attention to the detection of male sperm deformity rate and DNA fragmentation index, which can facilitate early personalized intervention and improve prognosis.
