    目的:观察实时剪切波弹性成像(Sensor Web Enablement,SWE)技术与二维超声技术在桥本甲状腺炎背景下甲状腺癌中的联合诊断效能。方法:选取我院2022年1月至2023年12月收治的80例甲状腺结节且伴桥本甲状腺炎患者,均行二维超声、SWE检查,分析二维超声与SWE联合诊断价值。结果:病理结果示80例患者共检出80个结节,其中良性52个,恶性28个;二维超声效能:灵敏度71.43%,特异度96.15%,阳性预测值90.91%,阴性预测值86.21%,准确率87.50%;SWE效能:灵敏度78.57%,特异度94.23%,阳性预测值88.00%,阴性预测值89.09%,准确率88.75%;联合效能:灵敏度92.86%,特异度98.08%,阳性预测值96.30%,阴性预测值96.23%,准确率96.25%;联合诊断灵敏度、准确率均>SWE>二维超声(P<0.05);二维超声联合SWE技术的曲线下面积(Area Under the Curve,AUC)最大。结论:二维超声联合SWE在桥本甲状腺炎背景下甲状腺癌患者中诊断效能较高,可同时开展二维超声与SWE技术检查,以提高甲状腺癌的检出。

    Abstract:

    Objective: To observe the combined diagnostic efficacy of real-time shear wave elastography (SWE) technology and two-dimensional ultrasound technology in thyroid cancer under the background of Hashimoto"s thyroiditis. Methods: Eighty patients with thyroid nodules and Hashimoto"s thyroiditis admitted to our hospital from January 2022 to December 2023 were selected and underwent two-dimensional ultrasound and SWE examination. The combined diagnostic value of two-dimensional ultrasound and SWE was analyzed. Results:The pathological results showed that a total of 80 nodules were detected in 80 patients, including 52 benign and 28 malignant; Two dimensional ultrasound efficacy: sensitivity 71.43%, specificity 96.15%, positive predictive value 90.91%, negative predictive value 86.21%, accuracy 87.50%; SWE efficacy: sensitivity 78.57%, specificity 94.23%, positive predictive value 88.00%, negative predictive value 89.09%, accuracy 88.75%; Joint efficacy: sensitivity 92.86%, specificity 98.08%, positive predictive value 96.30%, negative predictive value 96.23%, accuracy 96.25%; The sensitivity and accuracy of joint diagnosis are both greater than SWE and two-dimensional ultrasound (P<0.05); The AUC of two-dimensional ultrasound combined with SWE technology is the highest. Conclusion: The combination of two-dimensional ultrasound and SWE has a high diagnostic efficacy in patients with Hashimoto"s thyroiditis and thyroid cancer. Two dimensional ultrasound and SWE techniques can be performed simultaneously to improve the detection of thyroid cancer.
