    目的:探讨超声弹性成像(ARFI)联合血流参数对卵巢肿瘤良恶性鉴别诊断价值。方法:选取我院2022年1月~2024年12月收治的80例卵巢肿瘤患者,均行ARFI及彩色多普勒血流显像检查,并以病理检查为金标准,分为良性组和恶性组,对比两组剪切波传播速度(SWV)、血流参数[搏动指数(PI)、阻力指数(RI)],通过ROC曲线分析各参数单独及联合鉴别诊断价值;并对比恶性组不同国际妇产科联盟(FIGO)分期患者各参数的差异。结果:经病理检查,良性组53例,恶性组27例(其中FIGO Ⅰ期7例、FIGO Ⅱ期9例;FIGO Ⅲ期6例、FIGO Ⅳ期5例)。恶性组患者SWV值显著高于良性组,PI、RI值均低于良性组(P<0.05);ROC曲线显示,SWV值及PI、RI值均对卵巢良恶性鉴别有一定价值,曲线下面积均>0.70,联合鉴别诊断的曲线下面积(0.982)及敏感度(98.00%)显著高于三项参数单独检测(P<0.05)。卵巢癌患者中,随着FIGO分期增加,SWV值增大,PI、RI值变小,差异有统计学意义(P<0.05)。结论:超声弹性成像及血流参数SWV值、PI值、RI值联合鉴别诊断卵巢肿瘤性质的效能较好,且与卵巢癌FIGO分期存在关联。

    Abstract:

    Objective: To investigate the value of acoustic radiation force impulse (ARFI) elastography combined with blood flow parameters in differential diagnosis of benign and malignant ovarian tumors. Methods: Eighty patients with ovarian tumors admitted to the hospital from January 2022 to December 2024 were selected. All of them underwent ARFI elastography and color Doppler flow imaging. With pathological examination as the gold standard, the patients were divided into benign group and malignant group. The shear wave velocity (SWV) and blood flow parameters [pulsatility index (PI) and resistance index (RI)] were compared between the two groups. The ROC curve was used to analyze the value of each parameter and combination of parameters in differential diagnosis. The parameters of patients at different International Federation of Gynecology and Obstetrics (FIGO) stages in the malignant group were compared. Results: Pathological examination found 53 benign cases and 27 malignant cases (7 cases in FIGO stage I, 9 cases in FIGO stage II, 6 cases in FIGO stage III and 5 cases in FIGO stage IV). SWV of the malignant group was significantly higher than that of the benign group. PI and RI were lower than those of the benign group (P<0.05). ROC curve analysis showed that the areas under the curve of SWV, PI, and RI were greater than 0.70. The area under the curve and sensitivity of combined diagnosis (0.982 and 98.00%) were significantly higher than those of diagnosis with each parameter (P<0.05). For patients with ovarian cancer, as FIGO stage increased, SWV increased, and PI and RI decreased (P<0.05). Conclusion: The combination of ultrasound elastography and blood flow parameters (SWV, PI and RI) demonstrates good efficacy in identifying the nature of ovarian tumors. Besides, these parameters are associated with FIGO staging of ovarian cancer.
