    目的:比较血糖与糖化血红蛋白在糖尿病诊断中的敏感度与特异度。方法:选取2022年12月至2024年11月在三门峡市第三人民医院就诊的疑似糖尿病患者标本98例,进行空腹血糖(FPG)、餐后2h血糖(2hPG)及糖化血红蛋白(HbA1c)检测,并与在本院行健康体检的90例人员血糖水平进行对比,以此评估不同诊断方式的准确性况。结果:研究结果显示,血糖检测确诊阳性病例80例,阴性4例;糖化血红蛋白检测阳性87例,阴性8例,其诊断效能显著优于血糖检测,灵敏度、特异性及总体诊断符合率均明显提高,差异具有统计学意义(P<0.05);观察组患者血糖中各项指标均高于对照组,P<0.05。结论:HbA1c诊断糖尿病兼具高敏感度与高特异度,可作为首选筛查指标;联合FPG与2hPG可进一步提高检出率,推荐用于临床一线筛查。

    Abstract:

    Objective: To compare the sensitivity and specificity of blood glucose and glycosylated hemoglobin in the diagnosis of diabetes. Methods: We selected 98 specimens from suspected diabetes patients who visited the Third People"s Hospital of Sanmenxia City from December 2022 to November 2024. Fasting plasma glucose (FPG), 2-hour postprandial glucose (2hPG), and glycated hemoglobin (HbA1c) were measured, and compared with the blood glucose levels of 90 individuals who underwent health check-ups at the same hospital. This was done to evaluate the accuracy of different diagnostic methods. Results: The research results showed that there were 80 confirmed positive cases and 4 negative cases of blood glucose testing; 87 cases were positive and 8 cases were negative in glycated hemoglobin testing, and its diagnostic efficacy was significantly better than that of blood glucose testing. The sensitivity, specificity, and overall diagnostic accuracy were all significantly improved, and the difference was statistically significant (P<0.05). All indicators of blood glucose in the observation group were higher than those in the control group, with P<0.05.Conclusion: HbA1c has high sensitivity and specificity in the diagnosis of diabetes, and can be used as the first screening index; The combination of FPG and 2hPG can further improve the detection rate and is recommended for frontline clinical screening.
