    目的:分析CT灌注定量参数联合CT血管造影(CTA)评分预测急性缺血性脑卒中(AIS)的预后价值。方法:将2023年1月~2024年12月期间院内收治的罹患AIS患者共68例纳入本研究,收集各患者治疗后CTA评分及采集CT灌注定量参数[对比剂峰值时间(TTP)、对比剂平均通过时间(MTT)、脑血容量(CBV)、脑血流量(CBF)],并根据患者的预后情况分组为预后良好组(n=43)与预后不良组(n=25)。评估两方式联合应用于预测AIS患者预后的价值。结果:AIS患者出院前CTA评分与CBV、CBF水平呈正相关(P<0.05),与TTP、MMT水平呈负相关(P<0.05);CT灌注定量参数联合CTA评分预测AIS预后的AUC值较各指标单独预测更高。结论:采用CT灌注定量参数联合CTA评分可对AIS患者的预后进行准确预测。

    Abstract:

    Objective: To analyze the value of CT perfusion quantitative parameters combined with CT angiography (CTA) score on the prediction of prognosis of acute ischemic stroke (AIS). Methods: A total of 68 patients with AIS admitted to the hospital from January 2023 to December 2024 were included in this study. CTA score and CT perfusion quantitative parameters [time to peak of contrast agent (TTP), mean transit time of contrast agent (MTT), cerebral blood volume (CBV), cerebral blood flow (CBF)] were collected after treatment. According to the prognosis status of patients, they were divided into good prognosis group (n=43) and poor prognosis group (n=25). The value of the combination of the two methods on predicting the prognosis of AIS patients was evaluated. Results: The CTA score in AIS patients before discharge was positively correlated with CBV and CBF (P<0.05), and negatively correlated with TTP and MMT (P<0.05). The AUC value of CT perfusion quantitative parameters combined with CTA score in predicting AIS prognosis was higher than that of each index alone. Conclusion: CT perfusion quantitative parameters combined with CTA score can accurately predict the prognosis of AIS patients.
