带袢钛板固定术治疗踝关节骨折合并下胫腓联合损伤的研究

    目的:研究使用带袢钛板固定术在踝关节骨折合并下胫腓联合损伤中的治疗效果。方法:选取2020年7月-2024年7月期间于本院行踝关节骨折手术治疗的患者86例作为研究对象。根据患者的手术意愿将患者分为对照组(36例)与观察组(50例)。两组患者均接受内固定术治疗。对照组使用下胫腓螺钉固定,观察组使用带袢钛板固定。分析比较两组的手术相关指标、术后恢复情况、踝关节与后足功能的恢复情况(踝关节及功能AOFAS踝-后足评分)。结果:观察组和对照组在手术时间、术中出血量和住院时间上均无显著差异(P>0.05)。术后,观察组的部分负重时间显著短于对照组(P<0.05),但两组的完全负重时间、骨折愈合时间均无显著差异(P>0.05)。术后3个月 术后6个月 术后12个月,观察组的AOFAS踝-后足评分均显著高于对照组(P<0.05)。结论:踝关节骨折合并下胫腓联合损伤内固定术使用带袢钛板固定,不会延长手术时间,不增加术中出血量,且相较于下胫腓骨螺钉固定,能保证术后早期恢复,促进患者踝关节功能恢复。

    Abstract:

    Objective: To study the therapeutic effect of titanium plate fixation with loops in ankle fractures combined with inferior tibiofibular syndesmosis injuries. Methods: 86 patients who underwent surgical treatment for ankle fractures in our hospital from July 2020 to July 2024 were selected as the research subjects. The patients were divided into the control group (36 cases) and the observation group (50 cases) according to their surgical intentions. Both groups of patients received internal fixation treatment. The control group was fixed with inferior tibiofibular screws, while the observation group was fixed with titanium plates with loops. The surgery-related indicators, postoperative recovery conditions, and recovery conditions of ankle joint and hindfoot functions (AOFAS ankle-hindfoot score for ankle joint and function) of the two groups were analyzed and compared. Result: There were no significant differences between the observation group and the control group in terms of operation time, intraoperative blood loss and hospital stay (P > 0.05). After the operation, the partial weight-bearing time of the observation group was significantly shorter than that of the control group (P < 0.05), but there were no significant differences in the complete weight-bearing time and fracture healing time between the two groups (P > 0.05). At 3 months, 6 months and 12 months after the operation, the AOFAS ankle-hindfoot score of the observation group was significantly higher than that of the control group (P < 0.05). Conclusion: The internal fixation of ankle fractures combined with distal tibiofibular joint injury using a titanium plate with a loop does not prolong the operation time, does not increase the intraoperative blood loss, and compared with distal tibiofibular screw fixation, it can ensure early postoperative recovery and promote the recovery of ankle joint function in patients.
