    摘要:

    目的:探讨重症加强护理病房(ICU)院内感染病原学分布及耐药性和影响因素分析。方法:对上饶市人民医院2022年1月~2024年12月期间,ICU病房发生院内感染的80例住院患者分离出的病原菌及其耐药性进行回顾性分析,并分析其影响因素。结果:80例ICU院内感染者共检出病原菌126株,其中革兰阴性菌92株(占73.02%),革兰阳性菌34株(占26.98%)。金黄色葡萄球菌对青霉素耐药率达93.33%,表皮葡萄球菌对青霉素耐药率为100%。肺炎克雷伯菌、大肠埃希菌对氨苄西林耐药性强,铜绿假单胞菌对多种抗生素耐药率超88.00%;长时间住院、抗生素使用不当、侵入性操作是ICU院内感染及耐药性形成的主要因素。结论:ICU院内感染以革兰阴性菌为主,革兰阳性菌常见且耐药高,长时间住院、抗生素滥用及侵入性操作是感染及耐药的主要原因,需强化防控、优化抗生素使用。

    Abstract:

    Objective: To explore pathogens distribution, drug resistance and influencing factors of nosocomial infection in intensive care unit (ICU). Methods: A retrospective analysis was performed on pathogens and their drug resistance in the 80 patients with nosocomial infection in ICU of Shangrao People"s Hospital between January 2022 and December 2024, and the influencing factors of nosocomial infection were analyzed. Results: In the 80 patients, there were 126 strains of pathogens, including 92 strains (73.02%) of Gram-negative bacteria and 34 strains (26.98%) of Gram-positive bacteria. The drug resistance rate of Staphylococcus aureus to penicillin was as high as 93.33%, while drug resistance rate of Staphylococcus epidermidis to penicillin was 100.00%. The drug resistance of Klebsiella pneumoniae and Escherichia coli was high to ampicillin, and drug resistance rates of Pseudomonas aeruginosa to multiple antibiotics were >88.00%. Long stay in ICU, improper use of antibiotics and invasive operation were main factors of nosocomial infection in ICU and drug resistance. Conclusion: Gram-negative bacteria are main pathogens of nosocomial infection in ICU, while Gram-positive bacteria are common and have high drug resistance. Long hospital stay, antibiotics abuse and invasive operation are main reasons for infection and drug resistance. It is necessary to strengthen infection prevention and optimize the use of antibiotics.
