    目的:探究高速涡轮牙钻微创拔牙术治疗阻生智齿的疗效及安全性。方法:选择2023年6月~2025年1月在我院接受治疗的80例阻生智齿患者,按照治疗方法分为观察组(n=40)和对照组(n=40)。对照组采用传统锤凿劈冠法,观察组采用高速涡轮牙钻微创术,比较两组主要疗效指标、手术相关指标、术后疼痛、安全性。结果:与对照组比,观察组拔牙窝完整率更高(P<0.05),面颊肿胀程度、张口受限程度更轻(P<0.05),手术时间、骨愈合时间更短(P<0.05),术中出血量更少(P<0.05),术后24 h、48 h、72 h的视觉模拟评分(VAS)评分更低(P<0.05),且术后并发症总发生率更低(P<0.05)。结论:高速涡轮牙钻微创拔牙术治疗阻生智齿能提高拔牙窝完整率,减轻术后肿胀、张口受限和疼痛,缩短手术及骨愈合时间,降低并发症发生率,临床值得推广。

    Abstract:

    Objective: To investigate the efficacy and safety of minimally invasive tooth extraction using high-speed turbine dental drill in the treatment of impacted wisdom teeth. Methods: A total of 80 patients with impacted wisdom teeth treated at the hospital from June 2023 to January 2025 were selected for the study. According to the treatment method, they were divided into the observation group (n=40, minimally invasive tooth extraction using high-speed turbine dental drill) and the control group (n=40, traditional mallet and chisel techniques). The main efficacy indicators, surgery-related indicators, postoperative pain, and safety were compared between the two groups. Results: The intact rate of tooth extraction socket was higher in the observation group than in the control group (P<0.05). Cheek swelling and restriction of mouth opening were milder in the observation group (P<0.05). Surgical time and bone healing time of the observation group were shorter (P<0.05), and intraoperative blood loss of the observation group was less (P<0.05). The observation group had lower VAS scores than the control group at 24 h, 48 h, and 72 h after surgery (P<0.05). The total postoperative complication rate was significantly lower in the observation group (P<0.05). Conclusion: Minimally invasive tooth extraction using high-speed turbine dental drill can improve the integrity of tooth extraction socket, alleviate postoperative swelling, restriction of mouth opening and pain, shorten surgical time and bone healing time, and reduce complication rate in the treatment of impacted wisdom teeth.
