    目的:探讨抗凝血酶III(Antithrombin III,ATIII)活性在骨科创伤患者出血风险评估中的应用价值。方法:选取2022年11月至2024年12月期间本院骨科创伤中心收治62例患者作为研究对象。根据患者入院后24小时内是否发生临床显著的创伤性出血,将患者分为非出血组(非出血组)和出血组(出血组)。收集并对比两组的基线资料。检测并对比两组的实验室资料。采用t检验、χ2检验、Mann-Whitney U检验进行组间比较,Spearman相关分析评估相关性,Logistic回归分析筛选独立危险因素,受试者工作特征(ROC)曲线分析评估ATIII活性预测出血的价值。结果:S62例骨科创伤患者中,发生出血的患者有19例,纳入出血组(出血组);非出血患者有43例,纳入非出血组(非出血组)。血组组的ISS评分显著高于非出血组,ATIII活性显著低于非出血组,PT、APTT显著延长,FIB水平显著降低,D-二聚体显著升高(P均<0.001),HGB、PLT显著降低(P <0.05)。多因素Logistic回归分析显示,ATIII活性降低、ISS评分升高和D-二聚体升高是骨科创伤患者出血的独立危险因素。ROC曲线分析显示,ATIII活性预测出血的曲线下面积(AUC)为0.920(95%CI:0.858-0.982,P<0.001),最佳截断值为74.90%,灵敏度为89.47%,特异度为83.72%。结论:ATIII活性降低是骨科创伤患者出血的独立危险因素,且具有良好的预测价值。

    Abstract:

    Objective: To investigate the value of Antithrombin III (ATIII) activity in the evaluation of bleeding risk in patients with orthopaedic trauma. Methods: 62 patients admitted to the orthopaedic Trauma Center of our hospital from November 2022 to December 2024 were selected as the study objects. Patients were divided into control group (non-bleeding group) and study group (bleeding group) according to whether clinically significant traumatic bleeding occurred within 24 hours after admission. Baseline data were collected and compared between the two groups. The laboratory data of the two groups were detected and compared. t test, χ2 test and Mann-Whitney U test were used to compare the groups, Spearman correlation analysis was used to evaluate the correlation, Logistic regression analysis was used to screen independent risk factors, and receiver operating characteristic (ROC) curve analysis was used to evaluate the value of ATIII activity in predicting bleeding. Results: Among 62 orthopedic trauma patients, 19 cases of hemorrhage were included in the bleeding group (study group). A total of 43 patients without bleeding were included in the non-bleeding group (control group). In blood group, ISS score was significantly higher than that in non-bleeding group, ATIII activity was significantly lower than that in non-bleeding group, PT and APTT were significantly prolonged, FIB levels were significantly decreased, D-dimer levels were significantly increased (P <0.001), HGB and PLT were significantly decreased (P <0.05). Multivariate Logistic regression analysis showed that decreased ATIII activity, increased ISS score, and increased D-dimer were independent risk factors for bleeding in orthopedic trauma patients. ROC curve analysis showed that the area under the curve (AUC) of ATIII activity to predict bleeding was 0.920 (95%CI: 0.858-0.982, P<0.001), the best cutoff value was 74.90%, the sensitivity was 89.47%, and the specificity was 83.72%. Conclusion: Decreased ATIII activity is an independent risk factor for hemorrhage in orthopedic trauma patients, and has good predictive value.
