    目的:研究乳腺多模态影像检查评估乳腺癌腋窝淋巴结(ALN)及内乳淋巴结(IMLN)转移状况的临床价值。方法:选取2020年1月至2023年12月于会昌县妇幼保健院和赣南医学院第一附属医院乳腺疾病诊疗中心进行诊治的乳腺癌患者1000例,所有患者均行乳腺癌前哨淋巴结(SLN)活检术与基于乳腺功能学磁共振(MRI)、超声造影技术、乳腺钼靶的多模态影像检查,根据SLN检查结果将患者分为转移组与未转移组,比较两组患者一般资料与多模态影像检查结果,采用多因素logistic回归模型与受试者工作特征曲线(ROC)分析多模态影像检查对乳腺癌患者ALN、IMLN转移的诊断价值。结果:两组原发病灶最大径、临床分期、组织学分级、分子亚型有显著差异(P<0.05)。多因素分析显示,原发病灶最大径、临床分期、组织学分级、分子亚型均是乳腺癌患者ALN、IMLN转移的独立危险因素(P<0.05)。4项指标联合预测曲线下面积为0.900,高于单独检查,敏感度92.31%,特异度77.78%。结论:多模态影像检查评估乳腺癌患者ALN、IMLN转移具有较高的鉴别诊断价值,临床中具有应用价值。

    Abstract:

    Objective: To study the clinical value of breast multimodal imaging in evaluating axillary lymph node (ALN) and internal mammary lymph node (IMLN) metastases of breast cancer. Methods: A total of 1000 patients with breast cancer who were diagnosed and treated in the Huichang County Maternal and Child Healthcare Hospital and the Breast Disease Diagnosis and Treatment Center of the First Affiliated Hospital of Gannan Medical College from January 2020 to December 2023 were selected. All patients underwent sentinel lymph node (SLN) biopsy and breast multimodal imaging based on functional magnetic resonance imaging (MRI), contrast-enhanced ultrasound and mammography. According to the results of SLN biopsy, the patients were divided into metastasis group and non-metastasis group. General information and multimodal imaging results of the two groups were compared. Multivariate logistic regression model and the receiver operating characteristic (ROC) curve were used to analyze the diagnostic value of multimodal imaging for ALN and IMLN metastases of breast cancer. Results: There were significant differences in the maximum diameter, clinical stage, histological grade and molecular subtype of primary lesions between the two groups (P<0.05). Multivariate analysis showed that the maximum diameter, clinical stage, histological grade and molecular subtype of the primary lesion were independent risk factors for ALN and IMLN metastases in patients with breast cancer (P<0.05). The area under the curve of joint prediction using the four indicators was 0.900, which was greater than that of each indicator. The sensitivity and specificity of joint prediction were 92.31% and 77.78%. Conclusion: Multimodal imaging has high value in evaluating ALN and IMLN metastases in patients with breast cancer.
