多排螺旋CT三期增强扫描鉴别小细胞肺癌与非小细胞肺癌的价值

    目的:探讨与分析多排螺旋CT三期增强扫描鉴别小细胞肺癌(small cell lung cancer,SCLC)与非小细胞肺癌(non-small cell lung cancer,NSCLC)的价值。方法:2019年3月到2024年6月选择在本院诊治的肺癌患者112例作为研究对象,所有患者都给予多排螺旋CT三期增强扫描检查,记录CT特征并判断诊断价值。结果:在112例患者中,手术病理判断为非小细胞肺癌82例(非小细胞肺癌组),小细胞肺癌30例(小细胞肺癌组)。非小细胞肺癌组多平面重建、最大密度投影等三维重建图像清晰度评分与小细胞肺癌组对比差异不大(P>0.05)。非小细胞肺癌组的血管聚集征、分叶征、锯齿征等CT特征占比为30.49%、32.93%、31.71%,低于小细胞肺癌组的90.00%、90.00%、86.67%,差异有统计学意义(P<0.05),而非小细胞肺癌组的毛刺征占比高于小细胞肺癌组,差异有统计学意义(P<0.05);非小细胞肺癌组在延时期、静脉期、动脉期的CT值与小细胞肺癌组相比差异有统计学意义(P<0.05)。多排螺旋CT三期增强扫描判断为非小细胞肺癌81例,多排螺旋CT三期增强扫描鉴别非小细胞肺癌与小细胞肺癌的敏感性分别97.56%(80/82)、96.67%(29/30),总诊断准确性为97.32%(109/112)。结论:多排螺旋CT三期增强扫描在鉴别小细胞肺癌与非小细胞肺癌方面具有重要的临床价值,值得在临床上继续推广使用。

    Abstract:

    Objective: To investigate and analyze the value of multi-slice spiral CT in differentiating SCLC from NSCLC. Methods: From March 2019 to June 2024, 112 patients with lung cancer diagnosed and treated in our hospital were selected as research objects. All patients were given multi-slice spiral CT phase III enhanced scanning to record CT features and determine diagnostic value. Results: Among the 112 patients, 82 cases of non-small cell lung cancer (NSCLC group) and 30 cases of small cell lung cancer (small cell lung cancer group) were determined by operation and pathology. There was no significant difference in the resolution scores of multi-plane reconstruction and maximum density projection in NSCLC group compared with that in NSCLC group (P>0.05). The proportion of CT features such as vascular aggregation, lobular sign and zigzag sign in NSCLC group was 30.49%, 32.93% and 31.71%, which were lower than 90.00%, 90.00% and 86.67% in NSCLC group, and the difference was statistically significant (P<0.05). However, the proportion of burrs in NSCLC group was higher than that in NSCLC group. The difference was statistically significant (P<0.05). The CT values in the extended stage, venous stage and arterial stage of NSCLC group were significantly different from those in the small cell lung cancer group (P<0.05). There were 81 cases of non-small cell lung cancer diagnosed by multi-slice spiral CT phase III enhanced scan. The sensitivity of multi-slice spiral CT phase III enhanced scan in differentiating non-small cell lung cancer and small cell lung cancer was 97.56%(80/82) and 96.67%(29/30), respectively. The total diagnostic accuracy was 97.32%(109/112). Conclusion: Multi-slice spiral CT phase III enhanced scanning has important clinical value in differentiating small cell lung cancer from non-small cell lung cancer, and is worthy of further promotion and use in clinical practice.
