体外冲击波治疗网球肘的临床疗效及其对疼痛缓解的作用

    目的 网球肘患者存在明显的疼痛症状,会影响其肘关节功能及活动度,降低握力,本研究提出采用体外冲击波对网球肘患者进行治疗,以达到缓解疼痛、改善肘关节活动度和握力的目的。方法 根据纳入、排除标准筛选96例网球肘患者进行分组对照研究,分组方法为数表法,共分为两组,一组为对照组,有48例,采用局部封闭注射法治疗;一组为观察组,48例,在局部封闭注射法的基础上添加体外冲击波治疗。对比两组的临床治疗效果,包括血清疼痛因子水平、肘关节活动度、无痛握力及复发率。结果 血清疼痛因子指标有内啡肽(β-EP)、P物质(SP)、神经肽Y(NPY),观察组治疗后上述3项血清疼痛因子水平均低于对照组(P<0.05)。采用GEMREN骨科关节角度量角器对患者的肘关节活动度进行测量,观察组治疗后肘关节外旋、内旋、过伸、前屈的活动度均大于对照组(P<0.05)。采用上海曼欣WCS-100型的电子握力计对患者的无痛握力进行测量,观察组治疗后的无痛握力大于对照组(P<0.05)。随访6个月,观察组中有2例复发,复发率为4.17%,明显低于对照组的20.83%(10/48),差异有统计学意义(P<0.05)。结论 体外冲击波治疗网球肘具有良好临床效果,不仅能有效降低患者的疼痛因子水平,缓解其疼痛程度,还可增强患者的肘关节活动和无痛握力,并有效控制复发。

    Abstract:

    Objective Patients with tennis elbow have obvious pain symptoms, which will affect the function and range of motion of their elbow joints and reduce their grip strength. This study proposes to use extracorporeal shock wave therapy for patients with tennis elbow in order to relieve pain and improve the range of motion of the elbow joints and grip strength.Methods According to the inclusion and exclusion criteria, 96 patients with tennis elbow were screened for a grouped controlled study. The grouping method was the random number table method. They were divided into two groups. One group was the control group with 48 cases, which was treated with local injection of blocking agents. The other group was the observation group with 48 cases, which was treated with extracorporeal shock wave therapy on the basis of local injection of blocking agents. The clinical treatment effects of the two groups were compared, including the levels of serum pain factors, the range of motion of the elbow joint, pain-free grip strength and the recurrence rate.Results The indexes of serum pain factors include beta-endorphin (β-EP), substance P (SP), and neuropeptide Y (NPY). After treatment, the levels of the above three serum pain factors in the observation group were all lower than those in the control group(P<0.05). The GEMREN orthopedic joint goniometer was used to measure the range of motion of the elbow joints of the patients. After treatment, the ranges of motion of external rotation, internal rotation, hyperextension, and flexion of the elbow joints in the observation group were all greater than those in the control group(P<0.05). The Shanghai Manxin WCS-100 electronic grip dynamometer was used to measure the pain-free grip strength of the patients. After treatment, the pain-free grip strength in the observation group was greater than that in the control group (P<0.05). During the 6-month follow-up, 2 cases in the observation group had recurrence, with a recurrence rate of 4.17%, which was significantly lower than 20.83% (10/48) in the control group, and the difference was statistically significant (P<0.05). Conclusion Extracorporeal shock wave therapy for tennis elbow has a good clinical effect. It can not only effectively reduce the levels of pain factors in patients and relieve their pain, but also enhance the range of motion of the elbow joints and pain-free grip strength of patients and effectively control recurrence.
