    目的:探讨CT多平面三维重建技术对腰椎间盘突出症(LDH)的临床诊断价值。方法 回顾性收集2023年1月-2024年6月期间本院收治的112例LDH患者的临床资料。所有患者入院均接受过CT检查,并在CT检查后进一步接受CT多平面三维重建检查。以磁共振成像(MRI)检查结果作为诊断LDH的金标准,分析CT多平面三维重建技术对LDH的诊断价值。结果 经MRI诊断结果显示:112例LDH患者中,中央型28例,脱出型35例,椎间孔型33例,旁中央型16例;L2-L3节段突出17例,L3-L4节段突出40例,L4-L5节段突出43例,L5-S1节段突出12例。CT检出LDH患者椎间盘突出类型与MRI检查结果的一致性一般(Kappa=0.606,P<0.001);CT多平面三维重建检出LDH患者椎间盘突出类型与MRI检查结果的一致性较强(Kappa=0.939,P<0.001)。CT多平面三维重建检出LDH患者椎间盘突出节段与MRI检查结果的一致性较强(Kappa=0.961,P<0.001),且高于CT检出LDH患者椎间盘突出节段与MRI检查结果的一致性(Kappa=0.844,P<0.001)。CT多平面三维重建对LDH检出准确率为95.54%(107/112),对LDH突出类型检出准确率为95.54%(107/112),对LDH突出节段检出准确率为97.32%(109/112),均高于CT,差异有统计学意义(P<0.05)。结论 CT多平面三维重建技术在LDH诊断中具有较好的应用价值,且可有效评估患者椎间盘突出类型和突出节段,提高检出准确率。

    Abstract:

    Objective: To evaluate the clinical value of CT multi-plane three-dimensional reconstruction in the diagnosis of lumbar disc herniation (LDH). Methods The clinical data of 112 LDH patients admitted to our hospital from January 2023 to June 2024 were retrospectively collected. All patients underwent CT examination on admission and were further examined by CT multiplane 3D reconstruction after CT examination. Taking the results of magnetic resonance imaging (MRI) as the gold standard for the diagnosis of LDH, the value of CT multi-plane 3D reconstruction in the diagnosis of LDH was analyzed. Results MRI diagnosis showed that among 112 LDH patients, 28 cases were central type, 35 cases were prolapse type, 33 cases were intervertebral foramen type and 16 cases were paracentral type. There were 17 cases of L2-L3, 40 cases of L3-L4, 43 cases of L4-L5 and 12 cases of L5-S1. The type of disc herniation detected by CT in LDH patients was generally consistent with that by MRI (Kappa=0.606, P < 0.001). The type of disc herniation detected by CT multiplane three-dimensional reconstruction was consistent with that by MRI (Kappa=0.939, P < 0.001). The consistency of disc herniation segments detected by CT multiplane three-dimensional reconstruction in LDH patients was stronger than that detected by MRI (Kappa=0.961, P < 0.001), and higher than that detected by CT in LDH patients with MRI results (Kappa=0.844, P < 0.001). The detection accuracy of CT multi-plane 3D reconstruction was 95.54% (107/112) for LDH, 95.54% (107/112) for LDH protrusion type, and 97.32% (109/112) for LDH protrusion segment, all of which were higher than those of CT, and the difference was statistically significant (P < 0.05). Conclusion CT multi-plane 3D reconstruction technique has good application value in the diagnosis of LDH, and can effectively evaluate the type and protrusion segment of patients with intervertebral disc herniation, and improve the detection accuracy.
