经胸、经食管心脏彩超评估风湿性心脏瓣膜病患者心脏结构及瓣膜功能的价值分析

    目的:分析经胸、经食管心脏彩超评估风湿性心脏瓣膜病患者心脏结构及瓣膜功能的价值。方法:选取2023年1月~2024年1月在我院及南昌大学第二附属医院接受治疗的60例风湿性心脏瓣膜病患者,均行经胸心脏超声心动图(TTE)和经食管心脏超声心动图(TEE)检查,以手术结果作为金标准,比较TTE和TEE超声检查的常规结果并分析两种方法检测心脏结构[左心室肥厚、左心房增大、左心室增大],瓣膜功能(单纯狭窄、单纯关闭不全、狭窄伴关闭不全)的价值。结果:两种方法检测左室射血分数(LVEF)、左心房内径(LAD)、左心室舒张末期内径(LVEDd)、左心室收缩末期内径(LVESd)等常规超声指标差异无统计学意义(P>0.05),但以手术结果作为金标准,左心室肥厚、左心房增大、左心室增大等心脏结构异常及单纯狭窄、单纯关闭不全、狭窄伴关闭不全等瓣膜功能异常的检出率比较,TTE均较TEE显著高(P<0.05)。结论:与TEE相比,TTE用于评估风湿性心脏病患者心脏结构和瓣膜功能的价值更高,可更为准确的检出心脏结构及瓣膜功能异常。

    Abstract:

    Objective: To analyze the value of transthoracic echocardiography (TTE) and transesophageal echocardiography (TEE) in evaluating cardiac structure and valvular function in patients with rheumatic heart valve disease. Methods: Sixty patients with rheumatic heart valve disease who were treated in our hospital and the Second Affiliated Hospital of Nanchang University from January 2023 to January 2024 were selected. All of them underwent TTE and TEE. Surgical results was taken as the gold standard to compare the results of TTE and TEE, and analyze their value in detecting cardiac structure and valvular function. Results: Left ventricular ejection fraction (LVEF), left atrial diameter (LAD), left ventricular end diastolic diameter (LVEDd) and left ventricular end systolic diameter (LVESd) detected by the two methods were comparable (P>0.05). With surgical results as the gold standard, the detection rates of cardiac structural abnormalities (left ventricular hypertrophy, left atrial enlargement and left ventricular enlargement) and valvular function abnormalities (simple stenosis, simple regurgitation and stenosis with regurgitation) using TTE were significantly higher than using TEE (P<0.05). Conclusion: Compared with TEE, TTE can better help to evaluate cardiac structure and valvular function in patients with rheumatic heart disease, and can more accurately detect abnormalities in cardiac structure and valvular function.
