人工股骨头置换术与PFNA固定对年股骨转子间骨折患者髋关节功能恢的影响

    目的 探究老年股骨转子间骨折(IFF)患者应用人工股骨头置换术与近端方旋髓内钉(PFNA)固定的治疗效果。方法? 选取2021年8月-2023年9月期间于本院就诊的70例老年IFF患者作为研究对象。采用随机数字表法将患者分为对照组和研究组,每组各35例。对照组行PFNA固定术治疗。研究组采用人工股骨头置换术治疗。分析对比两组的围术期指标、髋关节功能(Harris髋关节功能评分)、疼痛程度[视觉模拟评分表(VAS)]及并发症。结果 研究组的手术时间、术中出血量、切口长度均比对照组显著提高;下床活动时间及住院时间均比对照组显著缩短(P<0.05)。术后3个月、术后6个月,两组的Harris髋关节功能评分均比术前显著提高,且术后6个月,两组的Harris髋关节功能评分比术后3个月的髋关节功能评分显著提高,且研究组术后3个月、6个月的Harris髋关节功能评分均比对照组显著提高P<0.05)。术后3d、术后2周,两组的疼痛评分均比术前显著降低,且术后2周,两组的疼痛评分比术后3d的疼痛评分均显著降低,且研究组术后3d的疼痛评分比对照组显著降低(P<0.05)。术后2周,两组的疼痛评分无显著差异(P>0.05)。研究组的并发症发生率显著低于对照组(P<0.05)。结论? PFNA内固定术与人工股骨头置换术均可有效治疗老年IFF,但人工股骨头置换术有助于缩短术后下床活动时间及住院时间,利于加快术后髋关节功能恢复,减轻疼痛感,减少并发症,安全可靠。

    Abstract:

    Objective To investigate the therapeutic effect of artificial femoral head replacement and proximal square rotation intramedullary nail (PFNA) fixation in elderly patients with intertrochanteric fracture (IFF). Methods 70 elderly patients with IFF who visited our hospital from August 2021 to September 2023 were selected as the study objects. The patients were divided into control group and study group by random number table, with 35 cases in each group. The control group received PFNA fixation. The study group was treated with artificial femoral head replacement. Perioperative indexes, hip function (Harris hip function score), pain degree [visual analogue scale (VAS)] and complications were analyzed and compared between the two groups. Results The operation time, intraoperative blood loss and incision length of the study group were significantly increased than those of the control group. The time of getting out of bed and staying in hospital were significantly shorter than those in control group (P < 0.05). At 3 months and 6 months after surgery, the Harris hip function score in both groups was significantly higher than that before surgery, and at 6 months after surgery, the Harris hip function score in both groups was significantly higher than that at 3 months after surgery. The Harris hip function scores of the study group at 3 months and 6 months after surgery were significantly higher than those of the control group (P < 0.05). Pain scores in both groups were significantly lower than those before surgery at 3 and 2 weeks after surgery, and pain scores in both groups at 2 weeks after surgery were significantly lower than those in 3 days after surgery, and pain scores in the study group at 3 days after surgery were significantly lower than those in the control group (P < 0.05). Two weeks after surgery, the pain scores of the two groups were not significantly different (P > 0.05). The complication rate of the study group was significantly lower than that of the control group (P < 0.05). Conclusion Both PFNA internal fixation and artificial femoral head replacement can effectively treat IFF in the elderly, but artificial femoral head replacement is safe and reliable, which can help shorten the time of getting out of bed and staying in hospital after surgery, accelerate the recovery of hip function, relieve pain and reduce complications.
