    目的:分析普通乳腺X线摄影(MG)鉴别乳腺肿块良恶性的诊断价值。方法:前瞻性选取2022年6月~2024年6月因乳腺结节在我院就诊的62例患者。所有患者入院后均接受MG检查及病理学检验。每位患者均给予常规MG检查及按特定投照体位的MG检查,以病理检验结果为金标准,评估两种MG检查方式的检验效能。结果:病理学检查显示恶性患者43例,良性19例;常规MG检查结果恶性39例,良性23例;特点投照体位MG检查显示恶性43例,良性19例。常规检查、特定投照体位检查与病理学诊断对照的敏感度为62.8%vs81.4%,特异度为36.8%vs57.9%,准确率为54.8%vs74.2%,阳性预测值为69.2%vs81.4%,阴性预测值为30.4%vs57.9%。特定投照体位MG检查的准确率显著高于常规MG检查(P<0.05)。特定投照体位MG检查所得图像甲级率显著高于常规MG检查(P<0.05);不合格率显著低于常规MG检查(P<0.05);两种方法图像乙级率无显著差异(P>0.05)。结论:基于特殊投照体位的MG检查在乳腺肿块良恶性的鉴别中具有良好的诊断效能,能显著提升图片质量,为临床提供科学、准确的图像根据。

    Abstract:

    Objective: To analyze the value of ordinary mammography (MG) in distinguishing benign and malignant breast masses. Methods: This prospective study included 62 patients who visited the hospital from June 2022 to June 2024 due to breast nodules. All patients underwent MG and pathological examination after admission. Each patient received conventional MG and MG in a specific projection position. Pathological results were used as the gold standard to evaluate the efficacy of the two MG methods. Results: Pathological results showed 43 cases with malignant nodules and 19 cases with benign nodules. Conventional MG results showed 39 cases with malignant nodules and 23 cases with benign nodules. The results of MG in a specific projection position showed 43 cases with malignant nodules and 19 cases with benign nodules. Compared with pathological results, the sensitivity, specificity, accuracy, positive and negative predictive values of conventional MG were 62.8%, 36.8%, 54.8%, 69.2% and 30.4%, which of MG in a specific projection position were 81.4%, 57.9%, 74.2%, 81.4% and 57.9%. The accuracy of MG in a specific projection position was significantly higher than that of conventional MG (P<0.05). The rate of grade A images obtained from MG in a specific projection position was significantly higher than conventional MG (P<0.05). The unqualified rate was significantly lower than that of conventional MG (P<0.05). The rates of grade B images obtained by the two methods were comparable (P>0.05). Conclusion: MG in a special position is efficient in distinguishing benign and malignant breast masses, and can significantly improving image quality, thus providing a scientific and accurate image basis for clinical practice.
