1.5T DCE-MRI参数联合血清CCL-18水平检测对卵巢癌的诊断价值

    摘要:

    目的:分析1.5T动态对比增强磁共振成像(Dynamic contrast enhancement magnetic resonance imaging,DCE-MRI)参数联合血清趋化因子配体18(CC-chemokine ligand18,CCL18)水平检测对卵巢癌的诊断价值。方法:回顾性收集2021年4月-2023年7月期间本院收治的76例卵巢癌患者的临床资料作为研究组,另根据1:1比例选取同期76例良性肿瘤患者的临床资料作为对照组。将研究组患者根据分期不同,分为Ⅰ期(30例)、Ⅱ期(21例)、Ⅲ期(14例)、Ⅳ期(11例)。分析对比两组入院时DCE-MRI参数及血清CCL-18水平;偏回归分析其水平与卵巢癌发生的关联性;对比不同分期患者入院时各指标水平并分析各指标水平联合检测对卵巢癌的诊断价值。结果:研究组入院时参数计算容积转移常数(Volume transfer constant,Ktrans)、速率常数(Rate constant,Kep)血管外细胞外间隙容积比(Extravascular extracellular volume fraction,Ve)及血清CCL-18水平均显著高于对照组(P<0.05)。Logistic回归分析显示,入院时高水平的参数Ktrans、Kep、Ve及血清CCL-18均为患者发生卵巢癌的危险因素(P<0.05)。入院时DCE-MRI参数及血清CCL-18水平在不同分期中比较,Ⅰ期<Ⅱ期<Ⅲ期<Ⅳ期患者(P<0.05)。入院时参数Ktrans、Kep、Ve及血清CCL-18水平联合诊断卵巢癌的(Area under the curve,AUC)为0.754,均高于各指标单独检测(P<0.05)。结论:DCE-MRI参数及血清CCL-18水平均与卵巢癌的发生发展相关,临床联合检测可提高对卵巢癌的临床诊断效能。

    Abstract:

    Objective: 1.5T Dynamic contrast enhancement magnetic resonance imaging (DCE-MRI) parameters combined with serum chemokine ligand18 (CC-chemokine Ligand18) were analyzed. The diagnostic value of CCL18 in ovarian cancer. Methods: The clinical data of 76 patients with ovarian cancer admitted to our hospital from April 2021 to July 2023 were retrospectively collected as the study group, and the clinical data of 76 patients with benign tumors were selected as the control group according to a 1:1 ratio. Patients in the study group were divided into stage I (30 cases), stage II (21 cases), stage III (14 cases) and stage IV (11 cases) according to different stages. DCE-MRI parameters and serum CCL-18 levels were analyzed and compared between the two groups at admission. The correlation between their levels and ovarian cancer was analyzed by partial regression. The diagnostic value of combined detection for ovarian cancer in patients with different stages was analyzed by comparing the level of each index at admission. Result: The Volume transfer constant (Ktrans) and Rate constant (Rate constant) were calculated when the study group was admitted to hospital. Extravascular extracellular volume fraction (Kep) and serum CCL-18 levels were significantly higher than those in control group (P < 0.05). Logistic regression analysis showed that high levels of parameters Ktrans, Kep, Ve and serum CCL-18 at admission were risk factors for ovarian cancer (P < 0.05). DCE-MRI parameters and serum CCL-18 levels were compared in different stages at admission, patients with stage Ⅰ < stage Ⅱ < stage Ⅲ < stage Ⅳ (P < 0.05). the Area under the curve (AUC) of Ktrans, Kep, Ve and serum CCL-18 in the diagnosis of ovarian cancer at admission was 0.754, which was higher than that detected by each index alone (P < 0.05). Conclusion: DCE-MRI parameters and serum CCL-18 levels are related to the occurrence and development of ovarian cancer, and clinical combined detection can improve the clinical diagnosis efficiency of ovarian cancer.
