    目的:探究光学相干弹性成像(OCE)在大鼠实验性口腔癌早期诊断中的应用价值。方法:选取健康雄性Wistar大鼠23只,随机划分为模型组(n=15)与对照组(n=8)。分别于实验第0、8、16、24周,运用OCE系统在体测定并计算两组大鼠腭部黏膜的杨氏模量;以病理结果为金标准,通过比对杨氏模量值差异并构建受试者工作特征(ROC)曲线,评估OCE的诊断效能。结果:模型组大鼠腭部黏膜成功复现了从正常、不典型增生至浸润性鳞状细胞癌的动态演变轨迹。OCE定量检测结果揭示,模型组黏膜的杨氏模量值随病变进展呈现出阶梯式的增高,从基线至第24周癌变递进式增加,高于同期的对照组(P<0.001)。ROC曲线分析表明,杨氏模量诊断口腔癌的ROC面积达0.924(95%CI:0.868—0.981),其诊断敏感度达到0.938,特异度为0.775。结论:OCE技术能够无创、实时、高分辨率地定量表征实验性口腔癌变过程中的组织生物力学特性演变,在口腔癌的早期无创诊断与筛查中具有良好的应用前景。

    Abstract:

    Objective: To explore the application value of optical coherence elastography ( OCE ) in the early diagnosis of experimental oral cancer in rats.Methods: Twenty-three healthy male Wistar rats were randomly divided into a model group (n=15) and a control group (n=8). At weeks 0, 8, 16, and 24 of the experiment, the Young"s modulus of the palatal mucosa in both groups was measured and calculated in vivo using the OCE system. Using pathological findings as the gold standard, the diagnostic efficacy of OCE was evaluated by comparing differences in Young"s modulus values and constructing receiver operating characteristic (ROC) curves.Results: The model group rats successfully reproduced the dynamic progression trajectory of oral mucosal lesions from normal to atypical hyperplasia to invasive squamous cell carcinoma. Quantitative OCE analysis revealed that the Young"s modulus values of the model group mucosa exhibited a stepwise increase with disease progression, progressively rising from baseline to week 24 and remaining significantly higher than the control group during the same period (P<0.001). ROC curve analysis indicated that the area under the ROC curve for diagnosing oral cancer using Young"s modulus reached 0.924 (95% CI: 0.868–0.981), with a diagnostic sensitivity of 0.938 and specificity of 0.775.Conclusions: OCE technology can quantitatively characterize the evolution of tissue biomechanical properties in the process of experimental oral carcinogenesis in a non-invasive, real-time and high-resolution manner, and has a good application prospect in the early non-invasive diagnosis and screening of oral cancer.
