    目的:探讨双面蓝光联合益生菌制剂对新生儿高胆红素血症的应用效果及对肠道菌群平衡的影响。方法:前瞻性选取我院2022年6月~2025年6月新生儿科收治的98例高胆红素血症患儿,依照1:1比例将其随机分为两组,各49例。对照组采取双面蓝光治疗,试验组采取双面蓝光联合益生菌制剂治疗。比较两组临床疗效,治疗前、治疗7d后采用钒酸盐氧化法检测血清总胆红素(Total bilirubin,TBIL)水平,计算胆红素下降速率,采用实时荧光定量PCR肠道菌群指标,比较光疗持续时间、黄疸消退时间及体重增长速率。结果:试验组总有效率为95.92%高于对照组81.63%(P<0.05);治疗后,两组血清TBIL降低,试验组低于对照组,胆红素下降速率升高,试验组高于对照组(P<0.05);治疗后,试验组双歧杆菌、肠道菌群多样性(Diversity of gut microbiota,Shannon)指数高于治疗前,试验组高于对照组,大肠杆菌低于治疗前,试验组低于对照组(P<0.05);试验组光疗持续时间、黄疸消退时间低于对照组,体重增长速率高于对照组(P<0.05)。结论:针对高胆红素血症新生儿采取双面蓝光联合益生菌制剂疗效显著,可提升胆红素代谢水平,改善肠道菌群结构及多样性,同时可缩短光疗持续时间,促进黄疸消退,提升患儿体重增长速率。

    Abstract:

    Objective: To investigate the application effect of dual sided blue light combined with probiotic preparations on neonatal hyperbilirubinemia and its impact on intestinal microbiota balance. Methods: A total of 98 children with hyperbilirubinemia admitted to the neonatology department of our hospital from June 2022 to June 2025 were prospectively selected and randomly divided into two groups in a 1:1 ratio, with 49 cases in each group. The control group was treated with double-sided blue light therapy, while the experimental group was treated with a combination of double-sided blue light and probiotic preparations. The clinical efficacy of the two groups was compared. Before treatment and 7 days after treatment, the serum total bilirubin (TBIL) level was detected by vanadate oxidation method, and the decline rate of bilirubin was calculated. The intestinal flora index by real-time fluorescent quantitative PCR was used to compare the phototherapy duration, jaundice subsided time and weight growth rate. Results: The total effective rate of the experimental group was 95.92%, which was higher than the control group"s 81.63% (P<0.05); After treatment, the serum TBIL of both groups decreased, with the experimental group lower than the control group. The rate of bilirubin decrease increased, with the experimental group higher than the control group (P<0.05); After treatment, the diversity index of bifidobacteria and gut microbiota (Shannon) in the experimental group was higher than before treatment, higher than the control group, lower than before treatment for Escherichia coli, and lower than the control group (P<0.05); The duration of phototherapy and jaundice resolution time in the experimental group were lower than those in the control group, and the rate of weight gain was higher than that in the control group (P<0.05). Conclusion: Double sided blue light combined with probiotic preparations has significant therapeutic effects on newborns with hyperbilirubinemia, which can improve bilirubin metabolism levels, enhance gut microbiota structure and diversity, shorten the duration of phototherapy, promote jaundice resolution, and increase the rate of weight gain in children.
